Order files upload missing radio button options?

Thank you for assisting me before - unfortunately my email was hacked so I need to get the fix for the radio button issue I experienced before. If you could contact me at This email address is being protected from spambots. You need JavaScript enabled to view it. and change my account to that email that would be great. Also I did not see my order for the fileupload plugin in my order history.

I just installed and enabled the plugin however I am not seeing the options for upload and email

I have searched for some options or suggestions to try with no luck can anyone point me in the correct direction - I am sure it is just a setting I am missing somewhere...
